1) Two copies of completed Application Forms;
2) Personal Statement;
3) Resume;
4) Two Reference Letters;
5) Graduation Certificate and Transcripts;
6) Copy of TOEFL score report (Original TOEFL report has been sent to you by ETS in October of 2000);
7) Certificate of Deposit issued by the bank of HSBC in xxxxx;
8) Certificate of CET-6;
